Information required at claim lodgement

To assist the client in qualifying, the Broker is to provide the following at claim lodgement:

  • policy number
  • confirmation that premium has been paid to your office or to Hollard within our credit terms
  • date of loss
  • location of loss
  • detailed description of loss including details of what has been damaged, lost or stolen
  • ABN & ITC% confirmed
  • EFT details must be provided
  • damaged property
    • Itemised quote or invoice for repairs, confirming the cause of damage
    • Photos of the damage
  • Lost or stolen property
    • Proof of ownership for items that have been lost or stolen
    • Replacement quote or invoice for equivalent items
    • Police report number
